How much are online programs publicly asking for compounded semaglutide? We can answer a narrower version of that question using the prices recorded in the SemaWise provider directory.

This October 2026 release reports the distribution of stated regular-price entries. The downloadable cohort is frozen as of October 14, 2026; later changes to the live directory do not rewrite this release. It does not measure what patients actually paid, compare identical prescriptions or represent every provider in the United States.

What does this snapshot show?

MeasureDirectory finding
All providers in the directory114
Entries classified as offering compounded medication102
Compounded entries with a recorded ongoing price47
Compounded entries excluded because the ongoing price is missing55
Lowest recorded regular-price figure$79
Lower quartile$149
Median$199
Upper quartile$239
Highest recorded regular-price figure$399
Included figures below $20034

These figures compare directory entries with different terms and inclusions. The median is a description of the recorded cohort, not a quote for your dose or a recommended price.

The recorded source-check dates for included entries span 2026-09-01 to 2026-10-14. These are the directory's verification dates; they do not mean every offer was checked on this article's publication day. Read the linked review's terms and check the current public offer before purchasing.

Read the method first

How was the cohort selected?

We start with the provider directory. Entries classified as brand-only are excluded from this compounded cohort. Of the remaining entries, we include only those with a recorded ongoing price and leave missing figures out. We do not replace a missing refill price with an introductory price, a guessed midpoint or a competitor's number.

The resulting cohort is non-representative. It reflects programs our directory covers and the subset publishing enough information for an ongoing-price entry. It is not a random sample and cannot tell us the average price paid nationally.

A program that offers both brands and compounds belongs in the compounded cohort when the directory classifies it as offering compounded treatment. That classification does not mean every product or prescription at the program is compounded.

How are the median and quartiles calculated?

Prices are sorted from lowest to highest. The site selects the values at the 25%, 50% and 75% positions using its existing cost-guide helper, rounding the position down. For an even-sized cohort, this reported median uses the lower middle value rather than averaging the two middle values.

No weighting is applied for patient counts, prescription volume, popularity, pharmacy size or a plan's duration. Each included directory entry contributes one recorded figure.

The semaglutide cost guide uses the same calculation on the current roster; its numbers can change after this frozen release. The price comparison board and individual reviews provide the plan details the aggregate cannot show.

Download the October 14, 2026 price observations as CSV. The dated export includes all 114 rows in this release, not just the compounded-price cohort. To reproduce the cohort, exclude brand-only rows and rows with a blank ongoing-price figure. Keep the full price terms and verification date beside every number.

Why are these prices not an equivalent-product comparison?

The ongoing-price field is a usable regular-price figure documented for the program. It does not normalize every plan to the same dose, supply length, payment schedule or services.

DifferenceWhy it changes the bill
Monthly payment versus prepaid termA low monthly equivalent can require months paid up front
Calendar month versus four-week supplyThe same displayed figure may mean different annual spending
Starting dose versus maintenance doseDose-dependent plans can rise later
Medication-only versus included clinical careSeparate visits or membership charges can add to the total
New-patient versus returning-patient eligibilityA listed rate may not apply to you
Formula and pharmacyDifferent preparations are not interchangeable prescriptions

For these reasons, multiplying the cohort median by twelve would give a hypothetical arithmetic total, not a typical patient's measured annual cost. Ask a provider for the actual invoice amount and refill terms for your prescription.

What does the index say about safety?

Nothing by itself. FDA says compounded GLP-1 products are not FDA-approved and has highlighted quality, dosing and inappropriate ingredient concerns1. A low price does not establish manufacturing quality, legal eligibility for compounding or medical suitability.

Use the pharmacy provenance register to see which programs name a dispensing pharmacy and what they disclose. That register measures disclosure, not a laboratory test of medication quality. Read compounded semaglutide vs Wegovy before making the brand-versus-compounded decision.

How should you use the snapshot?

Use it to recognize the spread of public figures, then compare a small number of plans at your actual dose. Confirm the medication, dispensing pharmacy, payment due today, refill schedule and total clinical charges.

A future release can report a later directory snapshot, but differences should not be called a market price trend without accounting for added providers, removed entries and changed terms. A new article date alone is not a new price collection.

A few more quick ones

Is this the average semaglutide price in the United States?

No. It describes stated regular-price entries in a non-representative provider directory. It is not a national survey or a measurement of patient spending.

Are introductory prices counted as ongoing prices?

Missing ongoing figures are excluded rather than replaced with introductory prices. Individual reviews explain any commitments, eligibility and dose conditions attached to included figures.

Can I compare the median directly with a Wegovy cash price?

Not as an equivalent-product comparison. The compounded cohort has different formulations, dose structures, terms and included services, and compounded medication is not FDA-approved.
