If you've just opened a box and you're staring at a red liquid, that's probably normal. Here's what's in it, what it does and doesn't do, and how to read the dose.
Why do compounders add B12 to semaglutide?
The usual pitch is energy and nausea. People eat a lot less on semaglutide, some feel tired, and B12 has a reputation as an "energy vitamin." Adding it makes a product feel like more than the brand, and it gives a program something to advertise.
There's a regulatory angle too. FDA has said it may treat a semaglutide-plus-B12 product as essentially a copy of approved drugs when both strengths are within 10% of commercial products given the same way1. In plain terms, adding B12 doesn't by itself make a compounded product legal.
Is semaglutide with B12 FDA-approved?
No. There is no FDA-approved product that combines semaglutide and B12. Every semaglutide-B12 vial is compounded, and FDA doesn't review compounded drugs for safety, effectiveness or quality before they're sold2.
FDA has also flagged the mix specifically. Its 2024 dosing alert noted that some compounders add cyanocobalamin (B12), pyridoxine (B6) and other ingredients, and said the safety and effectiveness of combining semaglutide with other ingredients has not been established3.
Does B12 help with nausea or fatigue on semaglutide?
There's no good evidence that it does. No trial has compared semaglutide with B12 against semaglutide alone, so any promise of fewer side effects is a guess.
The closest thing to data is one retrospective study of 94 people at a wellness studio who took a semaglutide-B12 injection. They lost about 4.6% of their weight in three months8. With no plain-semaglutide group to compare against, it can't show what the B12 added.
A 2026 review of compounded GLP-1 products found that about half of the 33 unusual products it identified added extras such as B12, glycine or niacinamide, and found little justification for adding nutrients7.
On energy, the National Institutes of Health is direct: B12 doesn't boost energy or endurance in people who already get enough from their diet4. It does matter if you're low. A real deficiency can cause anemia that leaves you tired and weak4.

My take as a patient: when I was worn out on semaglutide, it was nearly always because I'd eaten too little protein or drunk too little water that day. If you're dragging, start with semaglutide fatigue. If you think you might be low in B12, ask your doctor for a blood test rather than guessing. That matters more if you also take metformin, which can lower B12 levels4.

Why is my semaglutide red or pink?
Because of the B12. Cyanocobalamin, the form of B12 named in FDA's alert, is a dark red compound5, so even a small amount tints the liquid pink to red.
Plain semaglutide is different. Wegovy's label describes it as a clear, colorless solution6. So the color tells you something:
- Your vial says semaglutide with B12 and it's pink or red: that's expected.
- Your vial says plain semaglutide and it's colored: don't inject it. Call the pharmacy.
- Any vial that's cloudy or has particles: don't use it, whatever the color.
Three things to check on a semaglutide B12 label
- The semaglutide strength in mg/mL. It's the only number your dose math uses.
- The pharmacy's name. If it isn't on the label, ask before you inject.
- The look: pink to red is normal with B12; cloudy or particles are not.
How do you dose a semaglutide B12 blend?
The same way you'd dose plain compounded semaglutide. Only the semaglutide strength matters. A label might read "semaglutide 2.5 mg/mL, cyanocobalamin 0.5 mg/mL." Use the first number for your math and ignore the second.
On a U-100 insulin syringe, 100 units is 1 mL. Here's what common doses look like at three semaglutide strengths:

Units are computed from the semaglutide strength alone. Your prescriber sets the dose; this table only converts it.
That table is why a forum chart without a strength is useless: the same dose can be 10 units on one vial and 40 on another. Before every new vial, check the label and run your numbers through the semaglutide units-to-mg converter. If the strength on a refill differs from the last one, call before you draw.
Which programs sell semaglutide with B12?

Should you choose B12 or plain semaglutide?
If the price is the same and you don't mind the color, the B12 probably won't hurt you. Just don't pay extra for it expecting fewer side effects, because nobody has shown that it delivers.
