The answer matters because a measured low needs treatment, while the same sensations can also have other explanations. Your diabetes medicines, recent food intake and glucose reading are more useful than guessing from a symptom alone.
If someone is unconscious, having a seizure or unable to swallow safely, call 911. Do not give food or drink by mouth. Use prescribed glucagon if available and someone knows how to give it3,4. A severe episode is an emergency, whatever the glucose number happens to be.
Why your other diabetes medicines matter
The Ozempic label warns that combining semaglutide with insulin or an insulin secretagogue, such as a sulfonylurea, increases the risk of hypoglycemia, including severe episodes1. Wegovy carries the same combination warning. Neither the weekly injection nor the approved tablet removes the need to review those medicines2.
Tell the prescriber if you use insulin, glipizide, glimepiride or another medicine that can cause lows. Ask who will review your glucose results when semaglutide starts or increases. The label allows for clinician-directed changes to insulin or the secretagogue; it does not give every patient a standard amount to reduce. Do not change those doses on your own.
Can it happen without diabetes or insulin?
The risk should not be described as zero. Wegovy's label notes reported hypoglycemia in people without type 2 diabetes, while also explaining that its weight-management trials in this population did not systematically capture or report all hypoglycemia. Serious events were reported in its cardiovascular trial2. That evidence does not provide a reliable everyday rate for a person without diabetes.
Equally, “semaglutide lowers glucose” does not mean every headache, tired spell or hunger sensation is hypoglycemia. If episodes recur without a documented low, ask for assessment. A clinician can decide whether glucose testing or another investigation is appropriate. Our dizziness guide separates lightheadedness from several other patterns.
What symptoms should you recognize?
Low glucose can produce sweating, shakiness, hunger, dizziness, confusion or difficulty seeing or speaking. Some people have reduced awareness of their lows3. If you are at risk, agree in advance which symptoms or readings should trigger action and keep your treatment supplies accessible.
For many people with diabetes, below 70 mg/dL is the usual action threshold; your team may set an individual target. This differs from the below-54 mg/dL threshold used for certain clinically significant events in the Wegovy trial reporting2,4. Do not wait for the lower research threshold before following your diabetes plan.
How do you respond to a measured low?
For a person who is awake and can swallow safely, the ADA describes the 15/15 rule: take 15 grams of fast-acting carbohydrate, wait 15 minutes and recheck. Repeat if still low, following your care plan4. Glucose tablets or gel provide a measured option; read the package to identify the amount. A diet or zero-sugar drink will not supply the needed carbohydrate.
NIDDK gives a closely related 15–20 gram approach and advises a snack if the next meal is more than an hour away3. These are general diabetes rescue instructions, not a personalized semaglutide dosing plan. Ask your team which approach and supplies fit your situation, including any medicine that affects carbohydrate digestion. Get urgent help if you cannot keep treatment down or a low is not resolving.
Treat a low; plan for recurrence
- Check glucose when symptoms occur if you have a meter and an action plan.
- Awake and swallowing safely: follow the prescribed rescue plan; ADA uses 15 g then a 15-minute recheck.
- Unconscious, seizing or unable to swallow: no food or drink by mouth; call 911 and use prescribed glucagon if trained.
- Report repeated lows so the diabetes team can review the full medication regimen.
Prevent the next episode with a plan
Bring the glucose result, symptoms, medicine timing, meals and recent activity to your care team. Skipped meals, increased activity and alcohol can increase the risk of lows in people using glucose-lowering diabetes medicines3. Discuss days when nausea or vomiting makes eating difficult rather than assuming the same routine will always fit.
Use what to eat on Wegovy and alcohol on Wegovy or Ozempic for related planning questions. Semaglutide dosing and side effects explains treatment escalation; nausea remedies covers symptoms that may interfere with meals. The useful outcome is a written glucose and sick-day plan, including when to call, rather than a promise that lows cannot happen.
A few more quick ones
Can semaglutide cause hypoglycemia on its own?
It can occur, and the risk is not zero without diabetes. The clearest medication warning concerns use with insulin or sulfonylureas. Wegovy weight-management trials without diabetes did not systematically capture all hypoglycemia, limiting precise rate estimates.
What blood sugar reading is too low?
For many people with diabetes, less than 70 mg/dL is the usual level requiring action. Follow the target and rescue plan agreed with your care team; severe symptoms need help regardless of the reading.
Should I reduce insulin when I start semaglutide?
The prescriber may need to adjust insulin or a sulfonylurea to reduce low-glucose risk. There is no universal reduction to apply yourself. Arrange monitoring and instructions with the diabetes team.
Can I give juice to someone who has passed out?
No. Do not give food or drink to a person who is unconscious or cannot swallow safely. Call 911 and use prescribed glucagon if available and someone knows how to administer it.
