Knee pain can make activity difficult, while weight management can become harder when walking hurts. STEP 9 studied that combination in a defined group: people with obesity and moderate knee osteoarthritis. Its findings are encouraging, but the useful question is whether they fit your diagnosis and treatment goals—not whether semaglutide treats every kind of arthritis.

Who was included in STEP 9?

The randomized trial enrolled 407 adults with a BMI of at least 30, clinical and radiologic evidence of moderate knee osteoarthritis, and at least moderate pain. Participants received weekly semaglutide targeting 2.4 mg or placebo for 68 weeks. Both groups also received counseling about a reduced-calorie diet and physical activity1.

The average participant was 56 years old, average BMI was 40.3, and 81.6% were women. These details matter: this was not a trial of all people with knee pain, people without obesity, inflammatory arthritis or every stage of osteoarthritis. The findings should not be extended automatically to hip, hand or back pain.

How much did pain and weight change?

The trial used the WOMAC pain score, which runs from 0 to 100, with higher scores indicating worse pain. The average starting score was 70.9. Both groups improved, with a greater average improvement in the semaglutide group1.

Outcome at 68 weeksSemaglutide versus placebo
Average body-weight change−13.7% versus −3.2%
Average WOMAC pain-score change−41.7 versus −27.5 points
Average SF-36 physical-function change+12.0 versus +6.5 points

The pain changes are points, not percentages. These are group averages, not promised individual results. Physical-function improvement adds useful context, but a questionnaire result does not establish that every participant could resume every activity.

Permanent treatment discontinuation due to adverse events occurred in 6.7% with semaglutide and 3.0% with placebo; gastrointestinal disorders were the most common reason. Serious adverse-event incidence was similar between groups1. Benefit and tolerability both belong in the decision.

Does this mean semaglutide repairs cartilage?

No such conclusion follows from STEP 9. Its main results concern weight and symptoms; they do not establish cartilage regrowth or prove that treatment prevents knee replacement. Osteoarthritis affects the whole joint, and ACR describes treatment goals around reducing pain and improving function rather than an established cure3.

The trial also does not tell us that pain improvement occurred independently of weight loss. Avoid treating proposed anti-inflammatory mechanisms as a proven explanation of the clinical result. The semaglutide evidence guide explains why the product, population and measured outcome matter when interpreting studies.

Is Wegovy approved for knee osteoarthritis?

Knee osteoarthritis is not a current US Wegovy indication. The prescribing information lists specific uses, including weight management in eligible patients, with criteria that depend on the formulation and population2. A clinician may consider weight treatment in someone who also has knee osteoarthritis, but those are separate questions.

The STEP 9 regimen was weekly 2.4 mg injections. Do not assume the result establishes equivalent knee-pain benefits from oral semaglutide, a different dose or an unverified compounded preparation. Our Wegovy eligibility guide describes the weight-management assessment; a knee diagnosis alone does not establish eligibility.

What remains part of knee care?

ACR includes exercise, physical or occupational therapy, weight management when appropriate, assistive devices and individualized medicines or surgery among osteoarthritis options3. Semaglutide should not become a reason to abandon rehabilitation or delay reassessment of worsening symptoms.

Ask which movements fit your knee, whether supervised therapy would help, and how to build activity without assuming pain must be ignored. Our exercise guide provides broader context, while the knee clinician or therapist can adapt the plan to your function. Discuss nutrition and strength preservation as well; muscle changes during weight loss are a separate concern from the STEP 9 pain result.

Review pain medicines with your clinician. Ibuprofen during semaglutide treatment requires attention to your other medicines, kidney health and fluid losses; trial participation does not make any painkiller universally suitable.

What should you track at follow-up?

Choose practical goals such as walking to a shop, climbing stairs or sleeping with less pain. Track function and symptoms alongside weight and adverse effects. Ask when to reassess the diagnosis or seek a specialist opinion if progress is limited. If an operation is planned, tell the surgical and anesthesia teams about semaglutide early; our surgery guide covers that separate planning conversation.

A few more quick ones

Does semaglutide help knee osteoarthritis pain?

STEP 9 found greater average pain improvement with weekly semaglutide 2.4 mg than placebo in adults with obesity and moderate knee osteoarthritis. It does not guarantee an individual response or establish benefit for all knee pain.

Does it regrow knee cartilage?

The trial does not establish cartilage regrowth. Pain and function improvements should not be presented as proof of joint repair or prevention of knee replacement.

Is semaglutide approved specifically for osteoarthritis?

Knee osteoarthritis is not a current US Wegovy indication. A clinician may separately assess whether you qualify for an approved weight-management treatment.

Can I replace physical therapy with semaglutide?

Do not assume so. Exercise, rehabilitation and other individualized knee treatments remain part of care. Ask your clinician or therapist how weight treatment fits the full plan.
