Check Oak availabilityA map saying a state “covers semaglutide” leaves too much out. Ozempic for diabetes, Wegovy for weight management, and a compounded vial can lead to different answers for the same person.
This national guide separates insurance coverage from the ability to receive care. It includes two documented state examples, rather than marking every state yes or no without checking its current rules.
Which parts of semaglutide access depend on your state?
| Part of access | What decides it | What to check |
|---|---|---|
| Medicaid medication coverage | State policy, benefit and clinical criteria | Current preferred drug list and prior authorization rules |
| Commercial insurance | Your specific plan and covered indication | Formulary and weight-management exclusions |
| Medicare GLP-1 Bridge | Federal demonstration criteria | Part D enrollment and Bridge eligibility |
| Telehealth appointment | Rules where you are physically located | Clinician authorization to practice there |
| Medication delivery | Dispensing pharmacy and destination | Pharmacy licensure and shipping eligibility |
Do not assume that a provider serving your state means your insurance will cover its medication. Likewise, a covered drug does not mean every online program accepts your plan.
California: what changed in Medi-Cal Rx?
California's member notice says coverage for GLP-1 drugs used for weight loss only ended January 1, 2026. It separately describes continued access for qualifying other indications through prior authorization, and a weight-loss coverage pathway for members younger than 21 under EPSDT requirements1.
That means “Wegovy is excluded in California” is too broad. The member's age, prescribed use and approved authorization matter. Ask the prescriber which current rule applies to your situation.
North Carolina: why older coverage lists are wrong
NC Medicaid's February 2026 pharmacy newsletter says it reinstated GLP-1 weight-management coverage effective December 12, 2025, returning to its earlier criteria. Wegovy returned to preferred status; Zepbound and Saxenda returned as non-preferred options with requirements described in the newsletter2.
A September or November 2025 notice discussing the earlier cutoff does not tell the full story. Check the current NCTracks criteria before a new request or refill. The restoration also does not mean prescriptions are approved automatically.
What about the other states?
For a state not detailed above, use its Medicaid agency's current pharmacy pages and your managed care plan's information. Ask for all three items:
- The current drug list for your exact benefit.
- The prior authorization criteria for the drug and indication.
- The effective date and any continuation-of-treatment rules.
A national state tally is a snapshot. It can become stale when a budget change, new indication or reinstatement takes effect. Keep a copy of the rule your prescriber uses, including its date.
Check the rule that applies to you
- Match the medication to the diagnosis.
- Use the current policy for your exact benefit.
- Confirm the clinician and pharmacy can serve your location.
Does Medicare coverage vary by state?
The Medicare GLP-1 Bridge is a nationwide temporary program. It is separate from ordinary Part D coverage and has its own eligibility requirements3. You should not use a Medicaid state list to decide whether you qualify for Medicare access.
For commercial coverage, HealthCare.gov directs people to their insurer's covered-drug list and describes exception requests when a needed prescription is not covered4. The useful question is “does my plan cover this product for my diagnosis?” rather than “does my state cover semaglutide?”
Can an online provider treat you across state lines?
HHS says cross-state care can involve a full license, a compact, temporary practice rules or telehealth registration, depending on the jurisdiction5. Tell the program where you will be during the visit. This matters when you move, attend college elsewhere or take an extended trip.
Ask separately whether the dispensing pharmacy can ship to the address. Do not enter a different state to bypass an eligibility restriction.
What should you do if coverage ends?
Contact the prescriber before you run out. Ask whether the change is a benefit exclusion, an expired authorization or a request for more information. Those problems need different responses.
Use our Wegovy prior authorization guide to organize the paperwork. If cash payment is necessary, compare the current semaglutide cost routes and the total medication-plus-care bill. A cheaper compounded offer is a separate safety and regulatory decision, explained in compounded semaglutide vs Wegovy.
A few more quick ones
Does semaglutide insurance coverage depend on my state?
Medicaid policies and telehealth rules do. Commercial coverage also depends on the exact insurance plan and prescribed use, so a state alone does not settle the claim.
Does North Carolina Medicaid cover GLP-1 weight-management drugs?
Its February 2026 newsletter documents reinstatement effective December 12, 2025, with prior authorization and preferred-drug requirements. Verify current criteria for your benefit.
Is this a complete 50-state coverage list?
No. It explains how to verify coverage and presents documented California and North Carolina examples. It does not claim unverified coverage in other states.
