Metformin vs Semaglutide: How They Compare for Weight and Blood Sugar

If you are weighing metformin against semaglutide, you are really comparing two very different things that happen to overlap. One is a cheap, decades-old pill that has been a first-line diabetes treatment for a generation. The other is a newer, far more powerful injectable that produces weight loss on a scale metformin never has. The honest framing is not "which one wins" — it is understanding that they sit at opposite ends of a cost-versus-power trade-off, and that for many people they are used together rather than chosen between.

A grounding fact first. Metformin is an oral tablet, taken once or twice daily, that has been used to treat type 2 diabetes for decades — it has one of the longest safety track records in all of medicine2. Semaglutide is a GLP-1 receptor agonist, usually a once-weekly injection (also a daily pill, Rybelsus), sold as Ozempic for diabetes and Wegovy for chronic weight management. They both lower blood sugar and both can nudge weight down — but the size of those effects is where the comparison gets interesting. Weight loss: this is not a close contest

Here is the part marketing blurs and the honest answer makes clear: for weight loss, semaglutide is dramatically more effective than metformin. It is not a 10% edge — it is a different category of result.

In the STEP 1 trial, once-weekly semaglutide 2.4 mg produced about 14.9% mean weight loss over 68 weeks versus placebo in adults with overweight or obesity1. Metformin, by contrast, is associated with modest weight effects — typically a few pounds, often in the range of 2–3 kg, and frequently described as weight-neutral-to-mildly-favorable rather than a true weight-loss drug. Metformin is not, and has never been, FDA-approved for weight loss; its weight effect is a welcome side benefit of a glucose-lowering drug2.

So if the primary goal is meaningful weight loss, the evidence gap is large and one-directional. No honest reading of the trials puts metformin near semaglutide on the scale.

Blood sugar: both work, with different ceilings

For glycemic control the picture is closer, but still favors semaglutide on raw potency. Metformin is the long-standing first-line drug for type 2 diabetes — it lowers HbA1c reliably, is inexpensive, and has decades of outcome data behind it, which is exactly why guidelines start there2. Semaglutide typically produces larger reductions in HbA1c and body weight than older agents, which is why it is often added when metformin alone is not getting someone to target.

The practical reality in clinics is sequencing: many people start on metformin, and semaglutide is added on top when more glucose-lowering (or weight loss) is needed — not swapped in as a replacement. Often combined, not either/or

Cost and access: metformin's biggest advantage

This is where metformin decisively wins. It is a generic drug that costs only a few dollars a month, is stocked everywhere, comes as a simple pill, and almost never has supply problems. Semaglutide is expensive, can require prior authorization or run into insurance hurdles, has had periods of shortage, and — for the injectable forms — means weekly self-injection. For a lot of real-world decisions, the drug you can actually afford and reliably obtain matters more than the one with the bigger trial number. A theoretically stronger drug you cannot get or sustain is not, in practice, stronger for you.

Side effects and risk: similar headline, different fine print

Both drugs are gastrointestinal-heavy, which is worth knowing because it surprises people who expect them to feel different. Metformin commonly causes diarrhea, nausea, and abdominal discomfort, especially early on, and slow-release versions and gradual dose increases help2. Semaglutide's most common side effects are also GI — nausea, vomiting, diarrhea, constipation — clustering around dose increases and easing with slow titration, which is why the dosing-and-side-effects approach emphasizes starting low and going slow.

The risk profiles differ, though. Metformin carries a well-known caution about lactic acidosis in specific situations (significant kidney impairment, acute illness) and can lower vitamin B12 over the long term — but it has no weight-loss-class boxed warning. Semaglutide carries a boxed warning about thyroid C-cell tumors (based on rodent data) plus class cautions around pancreatitis and gallbladder disease. Neither is a casual drug; both are prescription-only and chosen and monitored by a clinician.

So which is "better"?

Honestly, it depends on the goal — and often the answer is "both."

  • For meaningful weight loss, semaglutide is far more effective; metformin's effect is modest12.
  • For first-line diabetes treatment on a budget, metformin's cost, simplicity, and decades of safety data are hard to beat2.
  • For added glucose-lowering when metformin isn't enough, semaglutide is commonly layered on top rather than replacing it.
  • For access and affordability, metformin wins clearly; the drug you can sustain beats the one you can't.
