Foundayo vs Wegovy Pill: Orforglipron vs Oral Semaglutide

For the first time there are two weight-loss pills to choose between. The Wegovy pill is oral semaglutide, the same drug as the Wegovy pen. Foundayo is orforglipron, a different molecule from Lilly that reaches the same GLP-1 receptor. Both start at $149 a month on self-pay, so the choice comes down to results, routine and side effects.

Here is the short version, then the detail.

The short version

What is Foundayo, and how is it different from the Wegovy pill?

The FDA approved Foundayo on April 1, 2026 for adults with obesity, or overweight with a weight-related health problem1. It is a small-molecule pill, not a peptide. That is why it survives your stomach without help and has no food or water rules2.

The Wegovy pill was approved on December 22, 20255. It is semaglutide, a peptide your stomach would normally break down. The tablet carries an absorption helper that only works on an empty stomach, so the morning routine is strict6.

That last row matters if you have known heart disease. The Wegovy pill carries an approved use to cut the risk of heart attack, stroke and cardiovascular death in that group5. Foundayo's label does not.

Which pill causes more weight loss?

On the numbers each company published, the Wegovy pill. But the two were never tested against each other for weight loss, so treat this as a strong hint, not a verdict.

Two things to keep in mind. The trials enrolled different people for different lengths of time. And the only analysis that lines them up statistically was written by Novo Nordisk staff: it estimated about 3 percentage points more loss on the Wegovy pill and fewer people quitting over side effects9. That points the same way as the raw numbers, but it comes from the company that makes one of the two pills.

The one head-to-head trial, ACHIEVE-3, did pit orforglipron against oral semaglutide. It was in type 2 diabetes, at the lower Rybelsus doses (7 and 14 mg), and its main measure was blood sugar. Orforglipron lowered A1C more, but stomach side effects and dropouts were also higher10. It tells you little about the 25 mg Wegovy pill for weight.

Do you have to take them on an empty stomach?

Foundayo: no. Take it once a day, with or without food, at whatever time suits you. Swallow it whole2.

Wegovy pill: yes, and the rules are strict6:

  1. Take it first thing in the morning on an empty stomach.
  2. Swallow it with no more than 4 ounces of plain water. No coffee, no juice.
  3. Wait at least 30 minutes before eating, drinking anything else or taking other pills.

From my own years on semaglutide, I can tell you the routine is where people slip. If your mornings involve coffee on the way out the door, or a thyroid pill that also wants an empty stomach, be honest with yourself before choosing the Wegovy pill. The pen and Foundayo don't ask for any of this.

How do you step up the dose?

Both pills start low and climb roughly monthly, which is how they keep nausea down.

Foundayo moves up only after at least 30 days on each dose, and your prescriber decides whether to go past 5.5 mg; 17.2 mg is the ceiling2. The Wegovy pill follows a fixed 30-day ladder to 25 mg6. If you miss 7 or more Foundayo doses in a row, the label says to restart at a lower dose2.

Which has more side effects?

Both cause the usual GLP-1 stomach trouble, mostly while the dose is climbing. Foundayo's label gives the clearest figures2:

  • Nausea: 26% to 35% on Foundayo, depending on dose, vs 10% on placebo.
  • Vomiting: 13% to 24% vs 4%.
  • Stopped because of side effects: 6% to 10% vs 3%.

On the Wegovy pill, 6.9% stopped because of side effects vs 5.9% on placebo, and the types of side effects matched the Wegovy pen6. In OASIS 4, stomach side effects of any kind hit 74% of people on the pill vs 42% on placebo8.

One Foundayo-specific catch: it can make birth control pills less reliable. The label advises switching to a non-oral method, or adding a barrier method, for 30 days after you start and after each dose increase2. Ask about this before your first prescription if it applies to you.

What does each pill cost?

Both companies sell the pills directly to cash payers, and the ladders line up almost dollar for dollar this year.

Two more numbers worth knowing:

  • List price. Foundayo lists at $649 for 30 days4, about half the $1,349 "original price" TrumpRx shows for the Wegovy pill11. That matters if your insurance charges a percentage of list.
  • Medicare. Lilly offers eligible Part D patients Foundayo for $50 a month through a Medicare GLP-1 Bridge program, from as early as July 1, 20263.

Which telehealth programs prescribe each?

Many online programs now list both pills alongside the pens. Ro prescribes brand-name Wegovy and the orforglipron pill through its membership, and Hims has moved to a brand-only menu that includes both pills. If you'd rather pick your own clinician, Sesame Care runs a marketplace that carries the Wegovy pill.

Is either pill cheaper than compounded semaglutide?

At the starting dose, sometimes. Compounded semaglutide programs we track charge a median of about $199 a month, with a range from $79 to $499. Which pill should you pick?

Think of it this way:

  • Pick the Wegovy pill if you want the larger weight loss seen in trials, if you have heart disease, or if you have already done well on semaglutide.
  • Pick Foundayo if a strict empty-stomach morning won't work for you, or if you take other morning pills that would clash with the 30-minute wait.

A few more quick ones

Is Foundayo the same as the Wegovy pill?

No. Foundayo is orforglipron, a non-peptide pill from Lilly. The Wegovy pill is oral semaglutide from Novo Nordisk. Both act on the GLP-1 receptor, but they are different drugs with different dosing rules.

Which pill works better for weight loss?

In separate trials, the Wegovy pill averaged about 14% loss at 64 weeks and Foundayo about 11% at 72 weeks. They have not been tested head to head for weight loss.

Can you take Foundayo with food?

Yes. Foundayo is taken once a day at any time, with or without food. The Wegovy pill needs an empty stomach, up to 4 ounces of water and a 30-minute wait.

How much does Foundayo cost without insurance?

As of October 2026, Lilly's self-pay price is $149 a month at 0.8 mg and 2.5 mg, $199 at 5.5 mg and $299 at higher doses, with the reduced prices running through December 31, 2026.
