Blood pressure can change while you take semaglutide, but two different questions are often mixed together: what happened on average in research, and what today's reading means for you. Trials generally found lower blood pressure with the weight-management injection. A high reading, a faint spell or an unexpected low still needs its own assessment.

A number on a cuff cannot tell you whether semaglutide caused it. Your recent readings, symptoms, other medicines and any vomiting or diarrhea help the clinician decide what needs attention. Keep taking prescribed blood-pressure medicines unless your care team instructs otherwise.

What did the trials find?

STEP 1 compared once-weekly semaglutide 2.4 mg with placebo alongside lifestyle intervention in adults with overweight or obesity without diabetes1. A later analysis used individual participant data from STEP 1, 3 and 4, covering 3,136 participants. The adjusted difference in systolic blood-pressure change favored semaglutide by 4.95 mmHg over the trials' 68-week periods2.

That is the difference between groups, not a promise that your top number will fall by five points. The analysis linked much of the reduction to weight loss and also documented changes in antihypertensive treatment. It was not a trial proving that a standard blood-pressure prescription can be replaced with semaglutide2.

These findings concern the 2.4 mg weekly injection. They should not be presented as an exact expected effect of the Wegovy tablet, Ozempic doses or every compounded preparation. Our heart-benefits guide covers a separate outcome: cardiovascular events rather than a cuff reading.

Can semaglutide cause low blood pressure?

Low blood pressure, drops on standing and fainting are recognized in Wegovy trial reporting. The label notes that some events occurred with gastrointestinal symptoms and volume loss, and low-pressure events were more frequent among people also taking antihypertensive medicines3.

That makes dizziness or feeling faint important to report, particularly during vomiting or diarrhea. It does not establish a universal need to reduce your blood-pressure dose. A clinician may need to review fluid status, readings and the full regimen. See dizziness on semaglutide and dehydration for those symptom patterns.

If you faint, are seriously unwell or have new chest, breathing or neurological symptoms, seek urgent medical help. Do not repeatedly stand up to test whether you still feel faint, or drive while dizzy. A routine home-monitoring plan is not a substitute for assessment during an acute illness.

What if your reading is high?

The average trial reduction does not rule out an individual high reading. Check measurement technique, record the result and contact your care team about readings that remain above the target they set. Do not take an extra pressure tablet or change semaglutide to correct a number on your own.

For a nonpregnant adult, AHA guidance treats a top number above 180 or a bottom number above 120 as severely high. If you have no new concerning symptoms, wait at least a minute and repeat; if it remains that high, contact a health professional immediately. With chest pain, shortness of breath, weakness, vision change, trouble speaking or other concerning symptoms, call 911 without waiting for it to fall5. Pregnancy needs its own urgent-care thresholds and plan; do not use this threshold to delay help.

Pregnancy requires its own care thresholds and plan. Symptoms can need emergency help without waiting for a particular cuff reading.

How should you check at home?

AHA recommends a validated automatic upper-arm monitor with a correctly fitting cuff. For a routine reading, avoid smoking, caffeine and exercise for 30 minutes, empty your bladder, rest quietly for five minutes, and support the bare arm at heart level. Take two readings one minute apart and record them4.

Ask your clinician when and how often to measure rather than checking compulsively after every sensation. Bring the monitor and record to an appointment so technique and accuracy can be reviewed. These instructions are for routine monitoring; they should not delay emergency care for severe symptoms.

Does a faster pulse mean higher blood pressure?

No single pulse reading establishes your blood pressure. Wegovy's label separately describes a rise in resting heart rate and recommends monitoring3. Our heart-rate guide addresses that different measurement. Tell the team about palpitations rather than assuming the cuff reading explains them.

For longer-term care, agree on a target, a home-check schedule and what readings or symptoms should prompt a call. Any medication reduction should come from that review. Kidney disease can also shape the plan; Ozempic for kidney disease explains a distinct indication without replacing blood-pressure management.

A few more quick ones

Does semaglutide lower blood pressure?

Average systolic pressure fell more with semaglutide 2.4 mg than placebo in weight-management trials. An individual participant analysis estimated about a 5 mmHg difference. This does not predict an individual response or replace antihypertensive treatment.

Can I reduce my blood-pressure medicine after losing weight?

A clinician may decide to adjust treatment after reviewing symptoms and readings. Do not reduce or stop it yourself because of weight loss, a trial average or one home measurement.

Does semaglutide cause high blood pressure?

Weight-management trials generally showed average lowering, but that does not explain or exclude a high reading in an individual. Record repeated readings, use correct technique and seek appropriate clinical advice.

What if my blood pressure is above 180/120?

For nonpregnant adults, either a top number above 180 or a bottom number above 120 needs attention. Without concerning symptoms, repeat after at least one minute and contact a clinician immediately if still very high. With concerning symptoms such as chest pain, breathing difficulty, weakness or speech/vision changes, call 911.

Are heart rate and blood pressure the same?

No. A pulse measures beats per minute; blood pressure measures pressure in the arteries. Wegovy can affect both, but one measurement does not establish the other.
